If you do use e-mail, then ASCII is fine. Don't bother generating fancy postscript, everyone can read latex-style equations. But please, do delimit the boundary between the cover letter and the report. And don't let the informality of the medium distract you from the formality and importance of your task!

Also, if you have the technology, please PGP encrypt e-mail to me. My public key is available via http, and on the PGP public key servers.

I'm not a stickler for forms. The standard review form, with its little checkboxes, is just to help you focus. The information it asks for is:

  • accept in present form, or with minor changes

    • article is so poorly written that contributions, if any, are obscure
    • article is not appropriate for reasons given in report
    • results are not correct
    • article is not suitable as a tutorial or survey for reasons given below
